DESIGN AND DEVELOPMENT OF A WEB-BASED EMPLOYEE PERFORMANCE APPRAISAL INFORMATION SYSTEM USING EXTREME PROGRAMMING METHOD(Case Study: UPTD Puskesmas Rawat Inap Cimalaka Sumedang)

UPTD Puskesmas Rawat Inap Cimalaka, as the technical implementation unit of the Health Office, is responsible for healthcare within its jurisdiction and has been conducting employee performance assessments since 2017. However, assessments are still conducted using paper forms, which pose risks of damage and loss. The aim of this research is to reduce delays and improve efficiency in managing employee performance assessments. The author conducted direct observations and interviews at UPTD Puskesmas Rawat Inap Cimalaka. To address this issue, a web-based employee performance assessment information system was designed. In this research, the author utilized Extreme Programming (XP) and Unified Modeling Language (UML) for system development, along with Hypertext Preprocessor (PHP) programming language. The outcome of this study is a web-based employee performance assessment information system capable of automatically generating employee lists and performance reports, thereby reducing delays and enhancing assessment efficiency. Based on user system testing results, it is concluded that the implementation of this web-based employee performance assessment system can prevent data damage and loss, as well as streamline the assessment process effectively and efficiently. This system supports better management of employee performance assessments at UPTD Puskesmas Rawat Inap Cimalaka.
